At the start of the game, the player can choose one of three playable healer characters: '''Dhanelu''', '''Artemi''', and '''Klaira'''. At the beginning of the game, the player can only play as either Dhanelu and Artemi: Klaira is unlocked after completing either Dhanelu's or Artemi's storyline. The unselected characters are also present within the storyline of Utopia. Utopia takes place over 12 in game days. Throughout the day, each character recieves quests from non playable characters: however, the story will continue regardless of whether they are completed or not, with the exception of the main quest on the first day for each character.

Each character has a group of NPCs that are called the "Bound". If the player does not complete their main quest during the day, one of their bound gets infected and dies. Regardless of which character the player chooses, they can also save the bound for the other playable characters, leading to a seperate ending.
